A GUIDE TO THRESHOLD THERAPEUTICS - CENTER FOR BIOVALENT® MANUAL THERAPY

Threshold Therapeutics, Inc. serves a twofold purpose as our home for treating individuals using Biovalent® Manual Therapy and as a training center for Frank’s classes and Mentorship programs. It is co-owned and directed by Frank Lowen, LMT, and Laurie Levine-Lowen, OTR/L, LMT. The following information has been designed to provide you with more detailed information regarding our clinical practice.

Frank, Laurie, the Associates, and visiting advanced practitioners provide complex-level therapy for local clients, and for many who travel quite a distance because of the reputation of this work. All students, clients and referrals are cordially welcome. Some students/practitioners prefer to accompany their patients to our practice. Observation provides an educational experience and may provide insights for further treatments, once the patient is home. Beginning in 2006, Frank is available for consultation in the clinic, but he is usually booked months in advance. Mainly, his time is devoted to creating and teaching his original body of work, Biovalent® Systems Manual Therapies.

Our normal hours of operation are 9:00 a.m. to 5:00 p.m., Monday through Friday, with lunch from 1:00 p.m. to 2:00 p.m. (Mountain Time Zone), except holidays and when Threshold Therapeutics hosts Biovalent® Systems Manual Therapy classes. Our Associates may be willing accommodate your needs before or after normal office hours by special request. Appointment requests may be made by telephone or by email.

THE ASSOCIATES

The Associates are all Biovalent® Manual Therapy advanced practitioners who choose to work as closely as possible with Frank Lowen and Threshold Therapeutics. Some Associates teach Frank's courses. Each has an independent practice under his/her own professional license and each brings their own unique skills and experience to the practice. Threshold Therapeutics and the Associates are a cohesive working unit. We often treat in the common room to share learning experiences. Clients may choose to receive therapy in a private room, but most prefer treatment in the common room, especially when Frank is available to consult.

PAYMENT FOR SERVICES

Manual Therapy and Movement Education is billed at the rate of $100 per hour, except when Frank is in the clinic (Apprentice Days), when the hourly rate is $125 per hour.

Our practice is a cash-based business, which means payment is due when treatment is rendered, via cash, check, Master Card or Visa.

APPRENTICE DAYS

Some of Frank's Teaching Apprentices live far away and have busy private practices. In order to advance their skills, they treat at Threshold Therapeutics when Frank is available. During these times, Frank floats in the clinic, lending input, consulting and assisting with treatment on a priority basis. We call these special sessions "Apprentice Days." Many out-of-town patients are scheduled at this time and may receive multiple hours (but no more than four per day) of therapy.

Cancellation Policy: When Frank Lowen is in the clinic, we require 24-hours advance notice per therapy hour booked to cancel an appointment without charge (e.g., 4 hours of treatment = 4 days' notice). During Apprentice Days, there is very high demand, and many patients must travel to accept an appointment. We will make every attempt to fill cancelled appointments; if we are successful in filling the appointment, no charge will be made.

Apprentice Days Therapy rate: $125.00 per hour

NON-APPRENTICE DAYS

Non-Apprentice Days Therapy rates:

Manual Therapy/Movement Education $100.00 per hour
Physical Therapy/Occupational Therapy (with physician's referral) $125.00 per hour

Cancellation Policy: A minimum advance notice of 24 hours is required to cancel an appointment without charge. Charges are based on treatment hours priced as described above. We will make every attempt to fill cancelled appointments; if we are successful in filling the appointment, no charge will be made.

INSURANCE BILLING

When scheduling appointments, please advise us if you require Occupational or Physical Therapy, to ensure that you are scheduled with an appropriate therapist. These requests may be difficult to fill as anticipated dates grow closer. Patients are responsible for payment to Threshold Therapeutics at the time of treatment, and can then file for reimbursement by their insurance carriers. Our office does not bill insurance companies directly, but we will provide invoices with billing codes for patients to submit to their insurers for reimbursement, provided the criteria for submission are met.

Laurie Levine-Lowen is a Licensed Massage Therapist and Occupational Therapist. Frank Lowen is a Licensed Massage Therapist. If a client has a physician's referral for Occupational or Physical Therapy, evaluation, documentation and invoices will be completed and treatment is billed at $125 per hour. For Physical or Occupational Therapy, Patients will be treated only for the diagnosis under that referral. When treating as a Licensed Massage Therapist, practitioners log and document sessions but will not perform any insurance documentation.

Illuminatis/Biovalent® Systems ( Provider Number P3337) is recognized by the Board of Certification, Inc. to offer continuing education (CE) for Certified Athletic Trainers (ATs) Category A CE.  ATs are responsible for claiming only those hours actually spent participating in the CE activity. BOC Approved Provider Number:  P3337.

Illumatinatis/Biovalent® Systems (Provider Number 447205-00) is approved by the National Certification Board for Therapeutic Massage and Bodywork (NCBTMB) as a Continuing Education Provider.
